Statewide Adoption & Permanency Network Services (SWAN)

We provide eligible at-risk youth with appropriate resources to foster the important permanency connections needed for a successful transition into adulthood.
Independent Living Program

We provide individual and group counseling and planning to assist young people in transitioning to a life of independence.
Adolescents Achieving Independence (AAI)

We provide out-client assistance to current or former foster care youth and young adults.
Achieving Independence Center (AIC)

We provide a safe, communal space where youth in foster care can find support as they strive toward attaining independence.
Transitional Living Program (TLP)

We provide semi-supervised independent living for homeless and runaway youth.
Thrive Program at the SASSO House

We provide housing and support for female-identifying trafficking survivors to reclaim their lives in the community.
Supervised Independent Living (SIL)

We support youth in finding an apartment, signing a lease, and obtaining housing-related services.
Room & Board (R&B)

We provide financial aid and casework support for older youth in foster care.
Rapid Rehousing (RRH) & Transitional Housing for Youth & Families

Rapid ReHousing (RRH) assists youth in locating safe housing, gaining employment, and developing the life skills to be self-supporting by allowing them to work closely with their Life Skills Counselor, Housing Stability Coach, and/or Mental Health Therapist.
The Synergy Project Street Outreach Program

We offer street outreach and assistance to runaway youth and youth experiencing homelessness.
