Transitional Age Bridge Program (TAY-Bridge)

Program Info

The objective of the TAY-Bridge Housing program is to provide case management and housing assistance services to Delaware County young adults ages 18-24 who were eligible for federal IV-E Chafee funded programming when they were under age 21. The TAY-Bridge program provides housing assistance, case management, and supportive services to unstably housed or homeless youth ages 18-24. Each youth referred to TAY-Bridge is assigned to a life skills case manager who rents the young adult client to the program, interviews them, and assesses each client’s housing, education, employment, life skills, referral needs and permanency needs and goals. TAY clients have access to a 24-hour on-call staff member. The life skills case manager provides ongoing support and guidance in order to ensure that each client achieves the goals outlined in their plan within a 24 month time frame. Goal plans are revised every 6 months. Rental assistance is also provided on a sliding-scale basis.
